How to Get an Illinois Medical Cannabis Card

Do you qualify for a medical cannabis card?

  • Are you at least 18 years of age (or are you a minor with an adult caregiver and physician certification from two physicians)?
  • Are you a legal resident of Illinois with proof of residency?
  • Have you been diagnosed with one of the qualifying conditions?
  • Has a physician certify that you have a qualifying condition, and have you established a bona fide relationship with that physician?

Opioid Replacement 

Starting January 31st, 2019 patients who would otherwise be prescribed an opioid can now choose to seek relief with medical cannabis and are eligible to apply for a 90-day medical cannabis card.

Patients who apply for a Medical Cannabis Registry Card on-line will receive provisional access to Verilife (you select your designated dispensary in the application) within 24 hours of completing the application process.

Qualifying patients who submit a paper application DO NOT receive a provisional registry card and must wait for their permanent registry card.

Frequently Asked Questions

If you already have a card, you only need to apply for a medical cannabis card extension. The Illinois Department of Public Health will mail you a renewal packet around the time of your expiration date. Simply return your renewal information a minimum of 45 days prior to the card’s expiration date. Your renewed card will be valid for two additional years. Patients who apply for a Medical Cannabis Registry Card online will receive provisional access to a licensed dispensary within 24 hours of completing the application process.

Qualifying patients who submit a paper application DO NOT receive a temporary registry card and must wait for their permanent registry card. If you are unable to submit your application, online all forms and materials should be sent to:

If you submit your application online will receive provisional access to a licensed dispensary within 24 hours of completing the application process.

If you submit your application by mail once you are approved, you will receive your medical cannabis card in the mail and can visit your designated medical cannabis dispensary. The lead time for mail applications is approximately 90 days.

Visit PharmaCannis. You must have your medical cannabis card and state-issued ID with you in order to purchase medicinal cannabis products.

Registered qualifying patients and their designated caregivers can obtain or possess 2.5 ounces of medical cannabis during any 14-calendar-day period. 

A patient may petition the Illinois Department of Public Health to increase the adequate supply by completing a waiver form with a check for $25.

The waiver must be completed by your physician.
